L-carnitine and acetyl-L-carnitine belong to the same carnitine system, but they are not the same molecule. Acetyl-L-carnitine, or ALCAR, is an acetylated ester of L-carnitine. In the body, both forms contribute to the pool of free carnitine and acylcarnitines.[1]

The carnitine system helps transfer fatty acids to mitochondria and maintains the balance of acyl groups and free coenzyme A. That is a basic biochemical function, but it does not make every carnitine form interchangeable in every study.[2]
A pharmacokinetic review reports that, after oral intake, ALCAR can be partly absorbed without hydrolysis and can also be converted to L-carnitine. Both forms have limited absorption and active renal reabsorption. Plasma concentration therefore does not automatically equal the amount in tissues.[1]
What the form name does not tell you
The word acetyl does not mean that ALCAR is simply a stronger version of L-carnitine. It is a different chemical form within the same system. Any claim depends on the exact form, population, study design, route and outcome measured.
If the question is material quality, both forms require correct identity, mass and purity checks. If the question is a research paper, first check whether it studied L-carnitine itself or ALCAR.
