Practical dosage and effect notes
In the phase 1 trial by Hunter and colleagues (BMC Musculoskeletal Disorders, 2010), four single intra-articular doses of BMP-7 were tested in knee osteoarthritis: 0.03, 0.1, 0.3, and 1 mg, 8 participants per cohort (3:1 active to placebo). No dose-limiting toxicity appeared at any level. Notably, the 0.1 mg and 0.3 mg doses showed a trend toward greater symptomatic improvement than placebo, while the 1 mg dose performed worse and came with more frequent injection-site pain.
That is a bell-shaped dose-response curve, where more is not better. It is why 0.1 mg and 0.3 mg in the table above are not an arbitrary pick but the upper edge of the effective phase 1 range, while the higher 1 mg level underperformed in the same trial. By week 12, both groups, placebo included, improved on pain by roughly 20%, which is worth keeping in mind when weighing the drug's own effect.
