Thymalin is a medicine and immunomodulator registered in Ukraine, a complex of polypeptide fractions isolated from the thymus (the thymus gland) of cattle. Per the official label it is given intramuscularly as part of combination therapy for conditions with reduced cellular immunity. Below is an honest review of tolerability, indications and how solid the evidence base under this compound is.

What Thymalin is and how it works
Thymalin belongs to the so-called thymic peptide bioregulators, preparations obtained from the thymus tissue of animals. In young age the thymus "trains" T-lymphocytes, and the idea of this class of agents is to use polypeptide fractions to support the maturation and differentiation of T-cells when the gland's own function is reduced (age, severe infection, trauma, burn, chemotherapy).
At the molecular level it is described that thymalin affects markers of T-lymphocyte maturation and the differentiation of haematopoietic progenitor cells. It is worth noting a caveat about the sources right away: most of these data come from the work of a single scientific school (the St Petersburg Institute of Bioregulation and Gerontology, the group of V. Khavinson), a significant share of the publications are old and Russian-language, and independent Western replication is limited. Indications (use)
Per the label, Thymalin is used as part of combination therapy for conditions accompanied by reduced cellular immunity, in particular:
- acute and chronic purulent-inflammatory processes (including of bones and soft tissue);
- burn disease, frostbite, trophic ulcers, long-non-healing wounds;
- states after radiation or chemotherapy, suppression of immunity and blood formation;
- prevention of infectious and purulent complications after operations, trauma.
The key phrase here is combination therapy: thymalin is not positioned as a standalone agent and does not replace causal treatment (antibiotics, surgery and so on) but is considered a supporting immunocorrective component at a doctor's decision.
Safety, contraindications and limitations
- Contraindications. The main one is individual hypersensitivity to the drug. The decision to use it during pregnancy, breastfeeding and in children is made by a doctor per the official label.
- Route of administration. Intramuscular only; the technical rules of asepsis for injections are mandatory.
- Not self-treatment. Since the drug affects the immune system, and the dose and appropriateness are determined by an immunogram and the clinical picture, self-prescribing is inappropriate.

What the evidence base actually says
Here candour is needed. Thymalin is an "old" drug of Soviet development (the late 1970s), and the main body of research around it:
- is concentrated in a single scientific school (Khavinson and co-authors);
- is mostly Russian-language and dated to the 1980s-2000s;
- contains a lot of work on animal models and small clinical series, rather than large multicentre double-blind RCTs (randomised controlled trial) of the modern standard.
Quality independent indexed RCTs at the level of evidence-based medicine under thymalin are scarce. This does not mean "it does not work"; it means the strength of the claims should be calibrated: the described immunomodulatory effects and mechanisms are plausible and published in peer-reviewed journals (see sources), but extrapolating them to a guaranteed clinical result is premature. So the honest frame is "a registered immunomodulator with an evidence base that is limited by modern standards," not "a proven agent from large RCTs."
The Longeva research context
Beyond the registered pharmaceutical, thymic peptides are of interest to researchers as a tool for studying T-cell maturation and immune ageing.
