Practical dosing and effect details
The STAMP-NAFLD trial (Cusi and colleagues, The Lancet Gastroenterology & Hepatology, 2021) compared three regimens: 250 mg once daily, 250 mg twice daily, and 500 mg once daily, all oral, over 12 weeks.
Across the full study population, PXL770 did not produce a statistically significant reduction in liver fat content versus placebo. But in the subgroup with coexisting type 2 diabetes (roughly 40–47% of each arm), the effect on liver measures and glycemic control was noticeably stronger than in the overall sample.
So instead of a simple higher-dose-stronger-effect pattern, the outcome here tracked which subgroup was being measured more than the milligram amount or the split-versus-single regimen itself. Researchers did not treat 250 mg twice daily as equivalent to 500 mg once daily even though the daily total is the same: for an oral drug, dosing frequency changes peak blood concentration in a way that an equal weekly total does not for a short-lived injected peptide.
