PXL770 is studied in metabolic research for its direct activation of AMPK, the enzyme pathway linked to fatty-acid oxidation and glucose regulation. It is not a peptide but a low-molecular-weight synthetic compound (a small molecule) for oral use, developed by the French company Poxel SA, which positioned it as the first-in-class direct activator of AMPK — a substance that binds the AMPK enzyme complex itself and activates it allosterically, unlike metformin or AICAR, which raise AMPK activity indirectly, by changing the cell's energy status.
AMPK (AMP-activated protein kinase) works as the cell's "fuel sensor": when the ATP/AMP ratio falls, it suppresses anabolic pathways, in particular de novo lipogenesis, and switches on fatty-acid oxidation. That is why direct AMPK activation has been considered a target in metabolic research for decades.
What the studies showed. In a phase 1b study in people with insulin resistance and NAFLD, PXL770 lowered de novo lipogenesis, so the target mechanism was confirmed in humans. However, the pivotal randomised, double-blind, placebo-controlled phase 2a study STAMP-NAFLD (120 participants, 12 weeks) did not meet its primary endpoint: the reduction in liver fat content versus placebo was not statistically significant (p=0.076 and p=0.084 for the two active regimens). This must be stated plainly: the compound did not "prove efficacy", it showed a signal that did not cross the significance threshold.
After that, Poxel effectively wound down the large NASH programme and refocused PXL770 on rare diseases, where the preclinical data looked stronger: autosomal dominant polycystic kidney disease (ADPKD) and X-linked adrenoleukodystrophy / adrenomyeloneuropathy. The FDA granted the compound orphan-drug status for ADPKD and Fast Track for X-ALD. These data remain mainly preclinical to date (cell and mouse models).
